Island Heart to Heart is an education and support program for people with heart disease or are at risk for heart disease and their families. Heart to Heart was developed to address the needs of people who are learning to live with heart disease.
Island Heart to Heart offers a series of seven classes where professionals, ie. Nurses, Cardiologists, Exercise specialist, Pharmacist, Psychologist, Social Worker, Nutritionist present information.
