Established in 2002, Sherbourne Health is a dynamic provider of integrated health services, community programs and capacity-building initiatives that enable people and communities to achieve wellness.
We're situated in Moss Park, a diverse community where many newcomers to Canada reside alongside a high number of people experiencing homelessness and precarious housing. We take pride in offering essential services to those facing barriers in accessing healthcare.

Health Services
Through our Family Health Team and Integrated Health Services, we offer comprehensive primary care by bringing together various healthcare professionals to coordinate treatment. Our team includes doctors, nurses, nurse practitioners, social workers, dietitians, and other experts who collaborate to provide personalized care. Additionally, we prioritize mental health by offering individual psychotherapy sessions with qualified staff specialized in serving our community's needs.

Thanks to our donors' generosity, we offer vital practical supports including water, hygiene supplies, socks, sleeping bags, blankets, takeaway lunch bags, and hampers. These items are essential components of the wraparound services provided by the Health Bus, WINK, and STASH programs.
Community & Capacity-Building Initiatives
Another large part of our work includes community and capacity-building initiatives. These programs have been created with the goal of uplifting the health and wellness of individuals and communities and include a range of options that includes multi-pronged approaches like our Food For Good program tackling food insecurity, all the way community support groups for chronic health conditions like diabetes.
