Sanguen is a not-for-profit, community-based health care agency where we believe to our core that people living in at-risk communities deserve compassionate support and accurate information about their health in order to make informed choices.

In the very beginning, Sanguen Health Centre started and continues to be a Ministry of Health funded Hepatitis C agency providing wrap-around support to those experiencing high degrees of marginalization as a result of their substance use, mental health, homelessness and chronic poverty. Our multidisciplinary team offers Hepatitis C nursing support and medical treatment, primary care support, Peer support and Outreach.

Sanguen Health Centre offers a person-centred practice of care, where service planning is specific to an individual’s desire for support. Our team uses Harm Reduction as its foundational pillar. Our philosophy is that there is value in every life.

About Our Community Health Van

While doing our work with hepatitis C, we’ve recognized one really important fact: people can’t focus on their health if they have other, bigger, important needs.

There are many agencies in the Waterloo Region doing incredible work to meet those needs  – but for people who aren’t already engaged with those agencies, or who experience barriers to accessing those services, things are much harder.

The Van, working in partnership with some amazing people, brings those necessary services to anyone who requires them. We know that when we reduce the barriers that stand between people and good health, every member of our community benefits.
