In consideration of my participation in the Lakeshore General Hospital Foundation’s fundraising Dragon Boat Race (further known as the “Event”), on Saturday, August 23, 2025 at 22Dragons located at 5524 Rue Saint-Patrick #150, Montréal, QC H4E 1A8, I agree as follows: That I knowingly will be participating in an Event that will require me to partake in water activities, including rowing and/or drumming a dragon boat on the Lachine Canal. The wearing of a personal flotation device is mandatory, and will be provided to me by 22Dragons. That I am aware of the risks associated with water activities, and I freely accept and fully assume all such risks and the possibility of personal injury, property damage, loss or death. That I am in good physical condition and have no medical or physical conditions that prevent me from safely participating in the Event. I assume liability for all risks that may incur as a result of participating in the Event. That the Lakeshore General Hospital Foundation, their board of director members, employees, volunteers or sponsors as well as 22Dragons, their owners and employees, will not be held responsible for any illness, injury or death as a result from participating in this Event. ASSUMPTION OF RISK AND RELEASE OF LIABILITY: Knowing the risks described above, and in voluntary consideration of participating in this Event, I hereby release from and will indemnify, and defend the Lakeshore General Hospital Foundation and their board of directors members, employees, volunteers and sponsors, as well as the owners and employees of 22Dragons, against any and all claim(s) which I have or may have for any losses, damages, injuries or death arising out of or in connection with my participation in this Event. I indicate that I have read the terms and conditions of participating and agree to abide by them. I have carefully read this Release Form and acknowledge that I understand it. No representation, statements, or inducements, oral or written, apart from the foregoing written statement, have been made. This Release Form shall be governed by the laws of the Province of Quebec. If any portion of this Release Form is held invalid, the rest of the document shall continue in full force and effect.
